We can significantly improve your skin by a variety of different measures that will make your skin look younger. There is a special type of treatment for all types of wrinkle of any depth that you may find irritating. When areas of the face or body lose volume or become flabby and creased due to age-related muscle relaxation, a positive change can be achieved by a lift, filling or tightening or a combination of all three methods using the latest technology. Deficiencies in volume and deeper wrinkles can be injected with suitable materials and “filled” in this way (see injection). Mimic facial wrinkles can be smoothed or even totally eliminated by the use of Botox (see Botox). Besides superficial wrinkles, pigment disorders (e.g age spots) can be successfully treated by surface ablation using peels, dermabrasion or laser (see surface ablation). In order to assess which treatment promises the best result for you personally, you will need to have a detailed consultation during which your questions and every possible rejuvenating measure can be discussed in detail and evaluated.
Skin rejuvenation with surface ablation
A number of factors, especially sunlight and smoking, cause typical ageing signs to appear on the skin over the course of time, including relaxation of the surface of the skin with formation of wrinkles and the appearance of pigmentation defects (e.g. age spots, hyperpigmentation). Targeted ablation of the superficial layers of skin encourages new skin cells to form, resulting in a fresher and more youthful appearance.
New skin rejuvenation technologies
Laser treatment
- Fractional laser
Unlike older laser systems, the fractional laser does not harm large areas of the skin’s surface. It lasers hundreds of small microscopic channels into the skin, purposely leaving areas in-between untreated; this produces rapid healing. Fresh new skin is formed from these tiny channels making the entire skin finer and getting rid of excess pigment from the upper skin layers. Treatment is almost painless and is carried out without anaesthetic. This system is very well suited to the treatment of acne scars when given in conjunction with fruit acid peels.
- Fraxel® re:fine-Laser
The newest skin rejuvenation procedure at the Rosenpark Clinic is Fraxel® re:fine, a skin rejuvenation process for treating creases, pigment spots and improving the structure of the skin on face, neck, decolleté and upper arms. The only side effect is redness that is visible for a few hours after treatment over the treated skin area before the skin becomes rejuvenated from the inside.
- Cool-Touch-Laser
By the effect of heat the laser shrinks the collagen fibres and smoothes the skin in the deeper layers of skin. A cool beam protects the surface skin layers from harm. This system is very useful for the treatment of acne scars.
Treating wrinkles with pulsed light
- Intense pulsed light (IPL)
Small dilated blood vessels on the face and irregular pigmentation due to light damage or the ageing process can be effectively and permanently removed today by intense pulsed light. This treatment also significantly decreases pore size and makes the entire skin appear smoother. Using special laser or intense pulsed light techniques, fine wrinkles can be smoothed away. No anaesthetic is required with this technique that is kind to the skin.
The energy from the intense pulsed light causes targeted damage and the long-term removal of small dilated blood vessels and irregular pigmentation. An added bonus is that fine wrinkles appear reduced and the entire skin is refreshed.
Chemical peel
By directly applying chemical substances (e.g. fruit acids, trichloric acid) the upper layer of skin becomes detached. The skin is rapidly renewed and appears refreshed.
Dermabrasion of wrinkles on the upper lip
Do you have small wrinkles on the upper lip or are your cheeks tending to wrinkle? If so, dermabrasion is suitable for you. We have had many years of experience in this area with classic dermabrasion (abrasion of the surface of the skin). Affected areas of skin are mechanically smoothed using a high-speed precision cutter. The new skin grows back with almost no wrinkles at all.
Pigment spots and moles
The increasing frequency of both benign and malignant skin changes represents a huge problem in our society. Our behaviour during leisure time and when on holiday in particular, with significantly more exposure to the sun causes greater damage and the risk of moles degenerating and becoming malignant. We are competent specialists in dermatology and would be please to assess any skin changes you may be worried about.
Treating wrinkles with injections
When wrinkles become deeper during the ageing process we treat them with a variety of different injectable materials such as poly lactic acid Sculptra®), Radiesse®, collagen, hyaluronic acid and fat grafting. The padding helps to reduce the depth of the wrinkles. Hyaluronic acid is particularly suitable for increasing lip volume.
Area of application
If the skin of the face shows somewhat deeper wrinkles and folds in some places, these can be stabilised by injecting certain materials which counteract the formation of wrinkles to some extent. This treatment is also suitable for areas of skin that have lost underlying fat during the ageing process (e.g. the cheeks).
Injectable materials
Non-permanent fillers (injection of wrinkles with degradable fillers)
At the Rosenpark Klinik we only use fillers in the treatment of wrinkles that can be broken down again by the body. Permanent fillers are associated with a loss of volume and thinner skin as one ages and sometimes years later can cause grotesque distortions, which is why we do not offer them here.
Details about the materials
Fat obtained through liposuction can be processed and injected back into wrinkles in the face. It can also be frozen for later use and thawed when required. This is particularly suitable for large-volume filling of cheeks that appear sunken, and pronounced circles around the eyes. However, even medium-sized and larger mimic folds can be filled directly using fat grafting. To obtain a long-lasting result, several treatments (3-5) should be given at 4-8 week intervals.
- Poly-lactic acid (Sculptra®)
Poly lactic acid is a substance that is produced naturally in the body and is synthetically manufactured for therapeutic purposes. The benefits include good tolerability and a sustained outcome. Sculptra® encourages new collagen to form in the skin. Sculptra® is particularly suitable for „nasolabial folds“ (wrinkles that run from the nose to the corner of the mouth) and to raise the corner of the mouth. As larger quantities can be safely used at during a single session, it is possible to fill large areas e.g. in the case of sunken cheeks. Normally treatment takes place over two to three sessions at 1-2 month intervals, then the effect obtained lasts for up to 18 months. When very diluted, Sculptra® can be effectively used to treat creases of the decolleté.
- Hyaluronic acid (Restylane®, Belotero®, Juvederm Ultra®)
Hyaluronic acid, which is also a substance produced naturally in the body (an “intercellular substance”) is produced biotechnologically. Hyaluronic acid products are very malleable and hence good for injecting fine wrinkles, e.g. those on the upper lip or for very smooth contours. Hyaluronic acid is ideal for contouring and enlarging upper and lower lips (lip enlargement). The substance is very well tolerated and is relatively rapidly degraded by the body. The effect normally lasts for between three and nine months.
- Collagen (Zyderm®I and Zyderm®II, Zyplast®)
Collagen is a fibrous component of connective tissue in the human body. The substance is obtained from closed stocks of American cattle and has been used as an injection material for almost 30 years. The collagen products are used for fine wrinkles and lip enlargement. The effect lasts for approximately the same time as hyaluronic acid.
- Radiesse®
is an injection material that smoothes out medium-sized and deep wrinkles in the face as well as loss of volume and the effect lasts a long time, though is not permanent. It is made from calcium hydroxylapatite microspheres in a carrier gel. The round microspheres form a framework in the deeper skin layers which encourages the body to form its own connective tissue. The carrier gel dissolves over a period of months and the structural microspheres, consisting of calcium and phosphate ions, are very slowly broken down by the metabolism. The effect of treatment lasts for approx. 2 years, i.e. significantly longer than that achieved following treatment with other degradable injectable materials. Radiesse® does not contain any animal ingredients and therefore no allergy testing is necessary.
Permanent fillers
The body is not able to break these down. Because of the known disadvantages such as allergy development and formation of nodules (granuloma) and the inability to adapt to natural, age-related changes in the structure of the skin, we do not use these fillers in our clinic.
Botox / Botulinumtoxin
The drug Botulinum toxin (commonly known as "Botox") is a „tension blocker“, which blocks the transmission from the nerve to the muscle of the command to contract for approx. 3-5 months. This drug, which has been used in medicine for over 30 years, helps spastic patients in particular and children with squints due to tense eye muscles. Botox has been used successfully in aesthetic surgery for almost 20 years. The treatment is particularly suitable for „frown lines“ on the forehead as well as „crow’s feet“ at the outer corners of the eyes, and wrinkles at the corner of the mouth that cause the mouth to droop.
Botulinum toxin / Botox smoothes the wrinkles of the skin by targeted relaxation of the underlying mimic muscles and regular repeat use (2-3 times p.a.) makes the skin appear significantly better.
Botox- areas of application in aesthetics
Continual conscious or even unconscious contraction of certain mimic muscles can have a negative influence on the face, particularly around the brow area and eyes, even though the patient feels well in him/herself.
Targeted application of Botox to the mimic muscles which cause pronounced wrinkles can block this effect (muscle block effect of Botox). The aim is to only treat those muscles that give a negative impression when they move (frown lines, drooping corners of the mouth, crow’s feet). Positive mimic muscles will then predominate in the overall facial expression and lead to a radiant natural expression.
Procedure
The drug Botulinum toxin / Botox is injected in very weak dosages into the region to be treated (crow’s feet, frown lines, drooping corners of the mouth).
The treatment leaves no trace except for a few very small puncture marks that are visible for a few minutes. It is recommended that this treatment be repeated after approx. 3-4 months as Botox is completely broken down by the body.
Surgical lifts
Facial tightening/face lift
When signs of natural ageing gradually become more obvious on the face and the external appearance no longer corresponds to one’s feeling of self-esteem, aesthetic surgery offers gentle and low-risk opportunities for correction. Patients typically state: “I look tired and unattractive but I don’t feel like that at all“. A face lift can quickly re-establish this feeling of self-esteem if other smaller measures such as Botox, injecting of wrinkles with fillers or laser treatments do not promise adequate success.
What do we understand by facial tightening and face lift?
A face lift is a tightening of the slackened structures under the skin (fat and muscle) and above this the tension-free spread of the previously loosened layer of skin (VIMOTIS-technique). This procedure enables the facial expression to be rejuvenated by several years. Face lifts are a suitable measure for deep cheek folds, drooping mouths and drooping cheeks. Almost invisible incisions (ask about the „Short Scar“ face lift) cause tightening and hence a significantly more youthful appearance. If one also has a double chin or excess skin on the neck, this can be elegantly removed by a small extension to the incision and liposuction of the chin area.
Various areas can be treated during facial tightening/a face lift:
The ageing process can favour different areas, however it frequently affects the entire face. Only by considering the face as a single unit and treating it as a whole can one successfully create the impression of a natural look that has not had any surgery.
Neck and cheek tightening
To obtain and maintain the natural and youthful radiance of the face, the dynamics of the face that were present previously must be re-established. What are dynamics? They describe the expression of our mimic muscles. These muscles lose their dynamics over the course of time and hence some of their expression. Compare this to a trampoline in which the tension in the canvas has become slack. The facial expression becomes sad, tired and tense. Tightening of the skin alone would rapidly give way due to the skin’s ability to stretch. Only by tightening and shortening the muscles can the original radiance be re-established. The patient wants a natural appearance that does not seem to have had any surgery. Re-establishing the dynamics of the face = rejuvenating, refreshing, only makes sense if this outcome can be permanent. The duration and maintainability of the results are critical to the success of the method. The discriminating patient is not satisfied with short-term attractiveness obtained by skin tightening alone.
The procedure is normally performed under a general anaesthetic or semi-narcosis. You have to remain in the clinic for a day after the operation. The stitches are removed on day 10 after surgery. The temporary facial swelling goes down after only a few days. Ideally, lymphatic drainage of the face should be carried out for approx. 3-4 weeks. Small bruises will have disappeared after approx. 10 days post-surgery. Very fine scars will no longer be visible after a few weeks. With the „Short Scar“ face lift in particular, you have the advantage of being able to wear any haircut as the scars will not be visible.
What can be achieved with surgery?
By means of neck/cheek tightening, the once harmonious proportions of the face that have been lost in the course of the natural ageing process can be re-established, and a more youthful, more vital radiance achieved. Sunken cheeks, pronounced nasolabial creases and pronounced wrinkling of the neck and cheeks can be tightened and significantly improved. Thanks to the modern and gentle surgical technique, a natural result is obtained that does not seem to have been operated on and which will last for years. If muscle bands are visible under the chin or there are wrinkles on the neck additional tightening of the neck will help achieve a harmonious balance between face and neck.
Forehead/eyebrow lift
What can be achieved with surgery?
Different aims can be achieved with forehead/eyebrow lifts. A forehead that is too high or too low can be individually shaped. If parts of the eyebrow are sunken, an eyebrow lift will make the eyes look “large and bright”. Smoothing the forehead will largely eliminate “frown lines”, vertical frown lines (thought lines) and „crow’s feet“.
What happens during surgery?
The incision is made either in the hair or at the hair line. Skin and fatty tissue are loosened from the layer of muscle and can now be lifted individually. Forehead wrinkles can be easily eliminated in this way and frown lines can be corrected during the same session. Following surgery a light compression dressing is laid over the area. As with face lifts, forehead and eyebrow lift patients are fit to return to society after only 1 week.
Tightening of the upper and lower eyelids in drooping eyelids and tear sacks.
What can be achieved with surgery?
Drooping eyelids or fatty deposits and excessive skin on the lower lids (tear sacks) which can arise when the tone of the elastic fibres is lost during the ageing process, makes the eyes appear old and tired and this upsets the harmony of the face. Sometimes this even causes the visual field to be considerably restricted, i.e. the area that the open eye can see at a glance. Eyelid tightening makes the eyes appear brighter and more open and eliminates the impression of tiredness and any impairment in the visual field. The contours of the eyes appear smooth again and natural-looking.
What happens during surgery?
During eyelid surgery the excess skin and fatty deposits, and slack muscle tissue also if necessary, are removed. To prevent lower lid inversion (ectropium) it is sometimes necessary to stabilise the outer corner of the lid (canthopexia – tightening of the eyelid). The fine skin of the eyelid is closed using the thinnest suturing material.
The procedure is normally done under a local anaesthetic so you can leave directly after surgery. The stitches are removed on day 5 after surgery. Small bruises and swelling of the eyelid disappear after approx. 10 days. After a few weeks the tiny scars are barely visible.
Body lifting
With too much surplus skin, body lifting is a good option to improve the figure. If the skin has been stretched after a drastic weight decrease or, for example, after pregnancy, it will no longer completely return to its original state in many cases. Also, age can also reduce the elasticity of the skin and affect the body form unfavourably. In this case, lifting in tumescence local anaesthesia helps. Areas frequently affected here are the upper arms, the inside of the legs or the abdomen. Also, leg or buttock is regularly carried out by us.
Even if liposuction does not lead to a sufficient tightening of the insides of the thighs, buttocks, tummy or upper arms, different types of body lifting in tumescence local anaesthesia or a full anaesthetic are available at your disposal, e.g. the upper arm lifting, inside leg lifting or abdominal wall tightening.
Lifting
After an extreme weight decrease, pregnancies or due to the aging process, natural changes can occur:
- Skin loses its elasticity
- Fat is redistributed
The consequence is a sagging of skin-fat according to the force of gravity: With arms outstretched, the upper arm portions sag, the connective and fatty tissue on the legs slips towards the knees. The belly and the buttock areas do not form a recess.
If after liposuction the sagging skin portions do not sufficiently form an outline themselves, this can be restored with a lifting operation. The surplus tissue is removed. The residual scars are positioned in areas, which are not noticeable.
One aim of an aesthetic lifting is the re-establishment of the outer appearance, giving an un-operated impression, and a long-lasting result.
Upper-arm lifting
If liposuction of the upper arms was not able to give the desired result and over-hanging skin and fat disturb the outer appearance, a lifting operation can help improve this.
What happens in the operation?
The incision runs from the shoulder to the internal elbow area. After lifting the surrounding skin, the skin is tightened and the surplus skin is removed. The amount of scar formation is very different, nevertheless it essentially depends on two factors:
- Condition of the connective tissue
- Continual movement in the operational area.
The operation is carried out in a general anaesthetic or also in local anaesthesia. The patient is monitored 24 hours and can go home the next day. 3-4 days of antibiotic protection and a special tape bandage are necessary. After 8 days, the patient can follow his/her vocational and social obligations again.
Abdominal lifting
In the case of sagging skin in the abdominal area, e.g. after several pregnancies or a big decrease in weight, the outer appearance can be changed favourably using an abdominal wall plastic. Before a possible operation, it is advisable to have terminated the weight reduction.
What happens with the operation?
After appropriate planning, an incision is made in the skin of the groin and the prominence of the symphysis pubis. The sagging skin-soft tissue is detached up to the costal arch and tightened downwards, the navel is cut around. Depending on the condition of the abdominal muscles, a tightening of the musculature is created by suturing (“internal corset”). Thereby, the waist is favourably accentuated. The surplus skin is shortened and the wound is closed. The covered navel is brought forward to its new position by making a small hole in the abdominal skin and sutured into place.
Leg and bottom lifting
If the elasticity of the flexible fibres of the skin diminishes, the skin and the subcutaneous fatty tissue lose their tone, usually first on the inside of the thigh. This is a normal change in the course of life and leads to fat depots on the inside of the thigh and the fold of the buttocks. If the desired result cannot be obtained with liposuction of the legs and over-hanging skin and fat tissue disturb, a lifting operation can improve the outer appearance.
What happens in the operation?
To start with (if not already happened), liposuction of the inside thighs is carried out in (TLA) tumescence local anaesthesia. Via a skin incision from the groin running into the fold of the buttocks, the sagged skin-fat area is exposed above the muscle fascia and tightened upwards. The surplus skin portion is removed.
Cellulitis treatment
The cause of cellulite lies in the special structure of female connective tissue. Cellulite results from fat deposits in the subcutaneous connective tissue. Full fat cells become lumpy stick like grapes together and swell up between the connective tissue strands.
With a special massage called Endermologie® (LPG®), it is possible it to activate the natural fat metabolism in such a way that the skin affected by cellulitis becomes supple and taut again. This massage is suitable also for pre- and/or subsequent treatment by liposuction and promotes the healing process. Newer procedures, like acoustic shock waves used in the medicine to destroy cells, are in the clinical trial phase with the goal of achieving a lasting positive result for cellulitis treatment. The Rosenpark Hospital is one of the few test centres for this new technology.
For lasting visible results, repetitive treatments are necessary.
